1. Purpose
This procedure outlines the process for applying for disability support and accommodations at Rosehill College. It ensures that students with disabilities receive the necessary support and accommodations to participate fully in their educational programs.
2. Scope
This procedure applies to all staff and students at Rosehill College involved in the process of providing or receiving disability support and accommodations. It covers the application, review, approval, and implementation of support measures for students with disabilities.
3. Definitions
- Disability: A physical or mental impairment that substantially limits one or more major life activities.
- Accommodations: Adjustments or modifications provided to students with disabilities to enable equal access to education.
- Applicant: A student who applies for disability support or accommodations.
- Disability Support Plan: A customized plan outlining the specific accommodations and support measures provided to a student with a disability.
4. Procedure
4.1 Application for Disability Support
- Complete Application Form:
- Form: Fill out the Disability Support Application Form available on the Rosehill College website or from the Student Service Officer.
- Supporting Documentation: Include relevant documentation such as medical reports or professional assessments that verify the disability.
- Submission: Submit the completed form and supporting documents online or to the Student Service Officer in person or via email at [email protected].
- Document Checklist:
- Ensure Submission: Check the documentation checklist before submitting the application to ensure all necessary documents are included.
- Formats Accepted: Accepted file formats include PDF, DOC, DOCX, JPEG, PNG, with a maximum file size of 10 MB per file.
4.2 Review and Approval Process
- Initial Review:
- Responsibility: The Disability Support Committee reviews the application for completeness and eligibility.
- Timeline: Conduct the initial review within 10 working days of receipt.
- Assessment Meeting:
- Meeting: Arrange a meeting with the applicant to discuss their needs and the accommodations required.
- Assessment: Evaluate the application based on the provided documentation and the discussion during the meeting.
- Develop Disability Support Plan:
- Plan: Develop a Disability Support Plan outlining the specific accommodations and support measures to be provided.
- Approval: Approve the plan within 5 working days after the assessment meeting.
- Notification:
- Method: Notify the applicant of the approval and details of the Disability Support Plan via email and through the online portal.
4.3 Implementation of Accommodations
- Communicate with Staff:
- Inform: Notify relevant teaching and support staff of the accommodations and adjustments required.
- Training: Provide training or guidance to staff as needed to implement the accommodations effectively.
- Monitor and Adjust:
- Review: Regularly review the effectiveness of the accommodations.
- Adjust: Make necessary adjustments to the Disability Support Plan based on feedback from the student and staff.
4.4 Appeals Process
- Submit Appeal:
- Form: Complete the Appeal Form if dissatisfied with the accommodations provided or the decision made.
- Timeline: Submit the appeal within 14 days of receiving the notification of the Disability Support Plan.
- Review Appeal:
- Committee: The Disability Support Committee reviews the appeal and makes a decision within 10 working days.
- Outcome: Communicate the final decision to the appellant in writing.
4.5 Documentation and Record Keeping
- Maintain Records:
- Applications: Keep accurate records of all disability support applications and decisions.
- Plans: Maintain copies of all Disability Support Plans and related documentation.
- Secure Storage: Store records securely for a minimum of 7 years.
- Confidentiality:
- Access: Restrict access to disability support records to authorized personnel only.
5. Responsibilities
5.1 Disability Support Committee
- Role: Review applications, develop support plans, and handle appeals.
- Responsibility: Ensure fair and effective management of disability support and accommodations.
5.2 Student Service Officer
- Role: Assist students with the application process, provide information, and coordinate support.
- Responsibility: Ensure students submit complete applications and facilitate communication between students and staff.
5.3 Teaching and Support Staff
- Role: Implement accommodations and provide feedback on their effectiveness.
- Responsibility: Ensure accommodations are provided as outlined in the Disability Support Plan.
6. Review Compliance
- Monthly Review:
- Meetings: Include disability support processes in the monthly Student Support Services Meeting agenda.
- Annual Review:
- Tool: Use the ASQA RTO Self-assessment tool template for the annual review.
- Audit: Include disability support processes in the annual compliance audit.
